Registration Requirements

1. Completed Registration Form (separated by division) (below)

2. Completed Registration Fee Payment (below)

3. Copy of child’s official Birth Certificate *(Name on all forms must agree with name on the birth certificate)

4. Copy of previous year – end of school year Report Card *(with all 4 semesters and final grades listed)

5. Recent Photo for ID Card
